PATIENT GUIDE
Admission
- Patient Admission must be made through the patient’s attending doctor that has obtained our Hospital Privileges.
- Patient must bring along his/her Lebanese Identity Card/Passport.
- For patients under the age of 18, please provide birth certificate. Parents of under-aged patients should also bring along their Lebanese Identity Card/ Passport.
- Patient requires to provide information, such as name, telephone number and Lebanese Identity Card/ Passport number, of a guarantor aged 18 or above.
Documents Required for Admission
Mandatory Documents for all patients:
- Patient official Identity document
- Hospital admission form filled and signed by the admitting physician
- Signed informed consent form
- Additional documents are required depending on the patient coverage and third-party payer
NSSF Patients
- Two copies of the patient’s Identity Card
- NSSF prior or post approval filled by the treating physician
- Social investigation “تحقيق اجتماعي “if any of the patient’s children is covered by the NSSF
MOPH and COOP Patients
- Copy of the patient Identity Card and one of his relatives
- Hospital admission form specific to Government Mutual Fund Request for hospitalization to be filled by the treating physician
- Guarantor’s approval
Lebanese Army Patients
- Copy of the medical Identity Card
- Medical report filled by the treating physician
- Guarantor’s approval
Internal Security Forces Patients
- Copy of patient Identity Card and Medical Identity Card
- Hospital admission template specific to Internal Security Forces
- Request templates (form 5) specific to the Internal Security Forces from the admissions’ office
- Guarantor’s approval
Self-Payers Patients
- Informed consent filled and signed by the physician and the patient or his legal representative
- Admission Form filled and signed by the physician
- Copy of patient Identity Card
Private Insurance Patients
- Copy of patient Identity Card and the insurance Identity Card
- Hospital admission form specific to the Guarantor
- Guarantor’s approval
Nursing Department
The Nursing Services is a fundamental department at Baakleen Medical Center dedicated to provide excellent Nursing care services to our patients.
The nursing staff faced many challenges such new technology, informatics, accreditation standards, COVID 19 recently, but we succeeded to cope to change in a positive way.
Nursing department provides both basic and specialized nursing care to the patients on 24hrs basis through the shift and or on call duty.
The department is one of the largest department directed by Nursing Director has 30 nurses distributed along with at different sections (Emergency Room, Medicine Floor, Operating Room, Maternity, Nursery, Chemotherapy, Polyclinic) each section has head of department.
One of the distinguished activities of the Nursing Service is to hold various Quality Assurance Programs. These reports are documented, reported and reviewed regularly.

- Nursing audits on different procedures
- Continuous education done by the nursing staff
- Competency testing done by the Registered Nurses and Practical Nurses
